Home oxygen therapy

Home oxygen therapy
Home oxygen therapy involves breathing in air that contains more oxygen than normal from a cylinder or machine in your home.

It may be prescribed if you have a heart or lung condition that causes low oxygen levels in your blood.

You can take oxygen in a number of ways using a:

tube positioned under your nose (nasal cannula)
face mask placed over your nose and mouth
tube placed into your mouth and down your windpipe
The tube or mask is attached to a ventilator machine.

How home oxygen therapy can help
If you have a health condition that causes low levels of oxygen in your blood, you may feel breathless and tired, particularly after walking or coughing. Fluid may also build up around your ankles.

Breathing air with oxygen increases the amount of oxygen in your blood.

This makes it easier to carry out activities that might otherwise be difficult, and helps reduce your symptoms.

Oxygen therapy also helps prevent damage to the heart and brain, which can be caused by low levels of oxygen in the blood.

It can help people with a range of health conditions, such as:

chronic obstructive pulmonary disease (COPD)
pulmonary fibrosis
heart failure
obesity-related hypoventilation
severe long-term asthma
pulmonary hypertension
conditions affecting the nerves and muscles or ribcage
cystic fibrosis
People who have oxygen therapy have different requirements. For example, you may only need oxygen for short periods during the day when you're walking about (ambulatory oxygen), or you may need it for longer periods during the day and night.

When oxygen therapy shouldn't be used
Oxygen therapy shouldn't be used to relieve breathlessness if your oxygen levels are normal.

This is because it can decrease your fitness level and cause a delay in finding out what's making you breathless.

Oxygen therapy assessment
If you have a long-term condition and your doctor thinks oxygen therapy might be helpful, you'll be asked to visit your nearest oxygen clinic for an assessment.

The amount of oxygen in your blood will be measured by taking a blood sample from your earlobe or wrist, or by attaching a sensor to your finger (a pulse oximetry test).

You may also be asked to breathe into a device called a spirometer during a lung function test. Regular oxygen treatment may be recommended if your blood oxygen level is low.

If you decide to have oxygen treatment at home, a healthcare professional at the clinic will help you work out how much oxygen you'll need and for how long. They'll also discuss the different ways you can take it.

Once you've agreed this with the clinic, they'll complete a home oxygen order form for you. It's similar to a prescription, and gets sent to the company delivering your oxygen and equipment.

You'll be asked to fill in a consent form, plus a form that asks a number of safety-related questions, such as whether you've had any falls recently. The forms are needed to make sure oxygen is installed in your home in the safest way possible.

The oxygen clinician will need to share some information about your requirements with the NHS and other key organisations, including the home oxygen supplier, your local fire and rescue service, and your electricity provider.

An engineer will visit your home to install the equipment, check that it's working properly, and explain how to use it. They'll also carry out a risk assessment and provide you with other important information, such as how to order oxygen refills.

Large oxygen cylinders
Oxygen cylinders will probably be prescribed if you only need oxygen for short periods to relieve attacks of breathlessness after an illness.

Oxygen is breathed in through a face mask or soft tube inserted into your nose (nasal cannula). You can talk, eat and drink while using a nasal cannula.

The use of oxygen cylinders should be regularly reviewed, and treatment should be stopped if your blood oxygen levels have returned to normal.

Oxygen concentrator machine
An oxygen concentrator machine is suitable if you'd benefit from having oxygen for many hours a day, including while you're asleep. It ensures you have a source of oxygen that never runs out.

The machine is about 75cm (2.5 feet) high and plugs into an electrical socket. It filters oxygen from the air in the room and delivers it through plastic tubes to a mask or nasal cannula.

Long tubing can be fixed around the floor or skirting board of your house, with two points where you can "plug in" to the oxygen supply. When the machine is installed, the engineer or nurse will discuss the length of tubing needed.

The machine is very quiet and compact. The engineer will explain how to use it and answer any questions you have.

A back-up oxygen cylinder will be provided in case there's a problem with the oxygen machine or there's a power cut. Regular maintenance checks will also be carried out to ensure the concentrator is always working properly.

Portable (ambulatory) oxygen
It may be possible to use a small, portable oxygen cylinder outside your home. This is called portable oxygen or ambulatory oxygen.

You'll need to be assessed to see whether you're able to use portable oxygen and whether it's likely to be beneficial.

Portable cylinders can deliver oxygen at a wide range of flow rates. Your oxygen healthcare professional will be able to advise you about the flow of oxygen you need. Regulators or conserving devices can be fitted to your cylinder so it lasts longer.

When full, portable oxygen cylinders weigh just over 2.3kg (5lbs) and hold just under two hours of oxygen (at a rate of 2 litres a minute).

Going on holiday
As long as you're well enough to travel and you plan in advance, you should be able to go on holiday while using oxygen.

Speak to staff at your local oxygen clinic as soon as possible if you're thinking about going on holiday, particularly if you want to go abroad.

They can advise you about what you need to do to stay safe while you're away. For example, you'll need to take your usual medication and a copy of your home oxygen order form.

Before travelling, you should tell the travel company that you use oxygen as they may need to make certain arrangements in advance. Travel insurance is essential.

If you're going on holiday in the UK, talk to your oxygen supplier to see whether it's possible for oxygen to be delivered to your destination. Try to give them as much notice as possible.

The British Lung Foundation website has more information and advice about going on holiday with a lung condition. You can also read more about what you need to consider before you travel.

Announcing the availability of Ayurvedic Coronil— the immunity-boosting medicine across the country from now on, yoga guru Swami Ramdev on Wednesday said that all issues with the Ayush ministry have been resolved and their ayurvedic medicine has been labelled as a ‘Covid-19 management drug’ by the ministry.

Reiterating that Patanjali Yogpeeth followed all due legal procedures in carrying out clinical control trials of Coronil, swasari and anu taila on Covid-19 positive patients, Swami Ramdev while addressing a press conference in Haridwar on Wednesday pointed out that after carrying out successful clinical control drug trials on asymptomatic and mild patients, Patanjali is also carrying out advanced stage research on serious patients.

“All issues with the Ayush department have been resolved and our ayurvedic drug has been labelled as a Covid-19 management drug. This is appropriate and proves that Patanjali has done nothing wrong but followed all due legal processes. Our Covid-19 immunity boosters are solely abstract and compounds of Ayurvedic herbs like Giloy, Tulsi, Ashwagandha and have been effective in the treatment of coronavirus patients. Soon this Coivd-19 immunity booster kit will be available across the country and we are in talks with those states also where due to some confusion the sale of Coronil was being opposed,” said Swami Ramdev.

He pointed out that right through from licence seeking process, clinical trials, scientific documentation to submission of all the requisite documents pre and post trials, Patanjali followed each and every rule but undue confusion and doubts were created to malign their sincere efforts on creating an immunity boosting Ayurvedic medicine for Covid-19.

Lashing out at those who cast aspersions on Patanjali’s Coronil, Swami Ramdev said that yoga and Ayurveda are ancient time-tested therapies and some segments of society including pro allopathic supporters, drug mafia and multi-national companies were not able to accept the remarkable work being done by Patanjali Yogpeeth in the field of health and medicine through Ayurveda.

“The drug mafia, MNCs and anti-national forces want me in jail,” he said.

Referring to Prime Minister Narendra Modi’s visit three years ago, during the inauguration of the Patanjali Research Institute, Swami Ramdev recalled that PM Modi had then suggested that Patanjali should establish Ayurveda as evidence-based medicine, for which Patanjali has been for the past few years concentrating immensely with a dedicated team of 500 senior Ayurveda researchers and practitioners working round the clock to achieve this target.

The time has come that Ayurveda should be given due medicinal status. We hail from a humble background, are seers, wear saffron clothes but our yoga-ayurveda based work and infrastructure is not going down well with some segments as a result of which we are being dragged unnecessarily into controversies,” said Swami Ramdev

Acharya Balakrishna, general secretary of Patanjali Yogpeeth, said that Patanjali has carried out clinical trials for more than ten diseases like dengue, hypertension, asthma, swine flu and it’s not the first time that they are conducting virology trials.

Centre asks Patanjali to stop advertising the drug, seeks details on formulation.
Hours after yoga guru Baba Ramdev unveiled an Ayurvedic medicine — ‘Coronil and Swasari’ — on Tuesday, claiming that clinical trials on COVID-19 affected patients had shown favourable results, the Central government asked Patanjali Ayurved Limited to stop advertising the drug and sought details on its claimed “successful trial and cure”.

In a statement issued by the Ministry of Ayush on the claims made by Haridwar (Uttrakhand)-based Patanjali Ayurved in treating COVID-19, the Ministry noted that it had taken cognisance of the news being recently flashed on its drug by the company. Facts of the claim related to the drug and details of the stated scientific study are not known to the Ministry, the statement said.

List of details
Patanjali Ayurved has been asked to provide, at the earliest, details of the name and composition of the medicines being claimed as successful in COVID-19 treatment; site(s)/ hospital(s) where the research study was conducted; the protocol, sample size, Institutional Ethics Committee clearance, Clinical Trials Registry-India (CTRI) registration, and results data of the study, and to stop advertising/publicising such claims till the issue had been duly examined.

The Ministry has also requested the concerned State Licensing Authority of the Uttarakhand government to provide copies of the license and product approval details of Ayurvedic medicines being claimed as useful in the treatment of COVID -19.

Regulated by Act
“Also, the concerned Ayurvedic drug manufacturing company has been informed that such advertisements of drugs including Ayurvedic medicines are regulated under the provisions of the Drugs and Magic Remedies (Objectionable Advertisements) Act, 1954 and Rules thereunder and the directives issued by the Central Government in the wake of COVID outbreak. The Ministry had also issued a Gazette Notification on April 21, 2020 stating the requirements and the manner in which research studies on COVID-19 with Ayush interventions/medicines should be undertaken,’’ the Ministry said.

‘100 patients’
Meanwhile, scientists associated with the trial said they had not yet published the results. Dr. Abhishekh Sharma, Assistant Professor of Medicine, National Institute of Medical Sciences, and among those who supervised the trial, said that 100 patients, who had tested positive for the virus, were recruited for the trial. Of them, 50 were given the medicine and five dropped out midway. The remaining 50 were administered a placebo.

“69% of the 45, when tested on the third day, were found clear of the virus and half of those on the placebo recovered,” said Dr. Sharma. That works out to 31 of those on the treatment tested negative, and 25 of those on placebo tested negative.

No other details on the profiles of the COVID-19 positive persons — the degree of sickness in those who enrolled for the trial, their age, whether they were on other medication prior to enrolment — were immediately available.

Clinical trial registry
A check on the clinical trial registry, where all trials must be registered, showed that a wide spectrum of patients were solicited. They were “asymptomatic, mildly symptomatic, moderately symptomatic and those aged 15-80”. The original aim of the study was to check the status of patients at day 3, day 7 and day 14, and whether they had improved immunological parameters due to the medicine. The medicine regime involved are the Tablet Swasari Ras (500 mg), Tablet Pure Ashwagandha Extract (500 mg), Tablet Pure Giloy Extract (500 mg), Tablet Pure Tulsi Extract (500 mg) and Anu Taila (nasal drop), according to information on the clinical trial website.

None of the medicines mentioned has ever undergone any form of clinical validation.
At a time when the number of novel coronavirus cases has crossed 7,700 — with deaths at 170 in China alone — and with the virus having spread to 20 countries/regions, the Ministry of Ayurveda, Yoga & Naturopathy, Unani, Siddha and Homoeopathy (AYUSH) has once again resorted to dishing out a dangerous prescription to people who might be infected with the virus.

In a press release circulated on Wednesday, the Ministry recommended certain Unani medicines for “symptomatic management of novel coronavirus” and certain Ayurveda and homeopathy medicines to help prevent coronavirus infection.

Nearly a month after China intimated the World Health Organization about the novel virus, no drugs to specifically target the virus are currently available and no candidate drugs are at any stage of clinical trial.

In the absence of specific drugs to treat the novel coronavirus, patients are only provided with symptomatic treatment, including making sure that patients get enough oxygen so that they survive long enough for the immune system to fight the disease on its own.

Not only is the recommendation by the Ministry highly irresponsible and dangerous but it also runs counter to the Ministry of Health and Family Welfare’s current practice of quickly isolating even suspected cases to prevent the spread of the virus and providing them with symptomatic treatment if found infected.

None of the medicines mentioned in the release has ever undergone any form of clinical validation. They have not been tested on people infected with the novel virus to study their ability to provide any kind of relief or prevent infection, either. Yet, the AYUSH Ministry has no hesitation in recommending the untested medicines to treat the virus, about which not much is known so far.

Symptoms not listed
Even as the Ministry issued a prescription to symptomatically manage infected people with Unani medicines, it has not cared to list out the symptoms that people infected with the novel virus exhibit or mention which medicines should be taken to treat which symptom.

People infected with the novel virus exhibit a wide range of symptoms. While most people exhibit milder symptoms akin to common cold such as fever and cough, about one in five manifest severe illness, including pneumonia and respiratory failure. Much like in the case of the Middle East Respiratory Syndrome caused by another coronavirus, some patients have been said to have had gastrointestinal symptoms, including diarrhoea.

By listing out the Unani medicines and asking people to use them for symptomatic management of the infection, the AYUSH Ministry is in fact encouraging people to self-medicate, which by itself is alarming. The press release is a mishmash of information put together in haste. While detailing Ayurveda medicines, it clearly states that the “advocacy is for information only and shall be adopted in consultation with registered Ayurveda practitioners only”, but no such cautionary note is found for Unani and homeopathy medicines.

With no evidence that the medicines would indeed provide even symptomatic relief, self-medication outside the healthcare system would put close contacts at grave risk of getting infected. After all, human-to-human transmission from infected cases to close contacts has been documented in China and other countries and even fourth-generation transmission has been recorded in Wuhan city, the epicentre of the disease outbreak in China.

Is the AYUSH Ministry aware of the kind of precautions that need to be taken while attending to people with the infection? Is it even aware that 16 healthcare workers who had been wearing all the mandatory protective gear while treating the infected cases fell ill themselves?

So when a passenger exhibiting symptoms suggestive of infection with the novel coronavirus is found at one of the 20 airports where thermal screening has been undertaken, will she be immediately quarantined and admitted to a hospital for further testing/treatment or will she be allowed to exercise her freedom to be treated at home with Unani medicines?

This should be reminder that the Health Ministry and AYUSH Ministry cannot afford to work at cross purposes.

City’s figures of the virus cases and deaths even sharper than Mumbai
Gujarat on Thursday recorded 324 infections and 20 deaths. The State’s cumulative numbers have increased to 9,592 cases and 586 deaths.

Full coverage on coronavirus

With the discharge of 191 patients after recovery, the number of the discharged stands at 3,753.

However, it should be mentioned that the recovery rate has shot up after the Central government’s new guidelines came. They don’t mandate retesting of the asymptomatic patients, who can be discharged after 10 days hospitalisation or treatment without testing if they don’t develop any symptoms.

As per the Health department’s figures, 5,210 patients are under treatment in various hospitals while 43 are critical and on ventilator support.

In Ahmedabad, 265 cases and 19 deaths were reported bringing the tally to 6,910 and 465. The city’s mortality rate is almost 6.7%, which is way above the national average of below 4%.

Ahmedabad’s figures of the virus cases and deaths are even sharper than Mumbai in proportion to the population of both cities.

The fact that despite stringent measures like complete shut down of the city except milk parlours and pharmacies in place, the virus is only surging without any containment with daily over 250 plus cases and around more than 20 deaths on average for the last 10 days in the city.

A controversy has erupted regarding number of testings being conducted. As per the data of the Health department, per day testings are being reduced.

On Thursday, only 2,412 tests were conducted against the capacity of 3,000 sample testings per day.

Moreover, the State government also disallowed the private laboratories to conduct the COVID-19 tests.

Shi Zhengli, a virologist renowned for her work on coronavirus in bats, said in an interview on Chinese state television that viruses being discovered now are "just the tip of the iceberg" and called for international cooperation in the fight against epidemics.
Known as China's "bat woman," the deputy director of the Wuhan Institute of Virology said research into viruses needs scientists and governments to be transparent and cooperative, and that it is "very regrettable" when science is politicized.

"If we want to prevent human beings from suffering from the next infectious disease outbreak, we must go in advance to learn of these unknown viruses carried by wild animals in nature and give early warnings," Shi told CGTN. "If we don't study them there will possibly be another outbreak."

Her interview with TV channel CGTN coincided with the start of the National People's Congress, an annual meeting of China's top leadership in Beijing. This year's NPC comes as the country's relationship with the U.S. turns increasingly frayed, with President Donald Trump and Secretary of State Michael Pompeo both saying the coronavirus sweeping the world is likely linked to the Wuhan laboratory.

China denies accusations COVID-19 was made in a lab

China's foreign ministry spokesman Zhao Lijian said on Thursday the World Health Organization has said there is no evidence that the coronavirus originated in a lab in the central Chinese city of Wuhan, where the epidemic first emerged in late 2019.

China has rejected the accusations. Shi has said that the genetic characteristics of the viruses she's worked with didn't match those of the coronavirus spreading in humans. In a social media post, she wrote she would "swear on my life" the pandemic had nothing to do with her lab. In another interview with CGTN over the weekend, the director of the Wuhan Institute of Virology, Wang Yanyi, said the idea that the virus escaped from the lab was "pure fabrication."

The outbreak has infected more than 5.4 million people worldwide and killed over 345,000.

Dopamine, Serotonin and Rines

Dopamine,  Serotonin and Rines

Dopamine is a messenger molecule in the brain that allows certain nerve cells to communicate with one another. Underestimated at its discovery, dopamine proved critical to central nervous system functions such as movement, pleasure, attention, mood, and motivation.

Dopamine is broken down into inactive metabolites by a set of enzymes—monoamine oxidase (MAO), catechol-O-methyl transferase (COMT), and aldehyde dehydrogenase (ALDH), acting in sequence.

A RIKEN research team has discovered an enzyme called Rines that regulates MAO-A, a major brain protein controlling emotion and mood. The enzyme is a potentially promising drug target for treating diseases associated with emotions such as depression.

Monoamine oxidase A (MAO-A) is an enzyme that breaks down serotonin, norephinephrine and dopamine, neurotransmitters well-known for their influence on emotion and mood. Nicknamed the "warrior gene," a variant of the MAOA gene has been associated with increased risk of violent and anti-social behavior.

While evidence points to a link between MAO-A levels and various emotional patterns, however, the mechanism controlling MAO-A levels in the brain has remained unknown.

Now, a research team headed by Jun Aruga at the RIKEN Brain Science Institute has shown for the first time that a ligase named Rines (RING finger-type E3 ubiquitin ligase) regulates these levels. Their research shows that mice without the Rines gene exhibit impaired stress responses and enhanced anxiety, controlled in part through the regulation of MAO-A levels. The study is published today in Journal of Neuroscience.

As the first study to demonstrate regulation of MAO-A protein via the ubiquitin proteasomal system, this research presents a promising new avenue for analyzing the role of MAO-A in brain function. Further research promises insights into the treatment of anxiety, stress-related disorders and impaired social functions.
